Grievance Procedure
You have a right to file a grievance if you believe you have been discriminated against. To file a grievance, you can speak with your case manager or any nursing staff member, complete a Patient Concern Notification form (staff will assist if needed), or contact the Patient Advocate.
You will receive a written response. If not resolved within 7 days, you will be notified of the expected resolution date.

If you believe that SJCH did not respond to any grievance you filed in a satisfactory manner, you may file a complaint with the United States Department of Health and Human Services, Office of Civil Rights at (800) 368-1019 or https://www.hhs.gov/ocr/about-us/contactus/ index.html.
